2. Add/Drop Form
This form is approved by the academic advisor and the dean.

Absence excuses are approved by the academic advisor and the Academic Advising Unit.

4. Retest Form
The Examination Committee and the faculty member decide on a suitable date for the retest.

5. Mid-Semester Progress Report
The academic advisor has access to the student's academic report and is responsible for preparing a mid-semester progress report for all of his/her advice.

6. Student Academic Report and Course Suggestion Form
The academic advisor is responsible for preparing the end-of-semester report for each student. This report contains the student's grades and a recommendation for the next semester's courses.

7. Complaint/Appeal Form
Refer to the previous section for possible complaints. For example, a student can file a complaint about a test score and the Examination Committee will decide whether the test needs to be re-examined based on their complaint.
